Medical Release Form: All players are required to complete and submit the Medical Release form to their team's Manager. Players will not be permitted to participate without submitted a completed Medical release form.


School Enrollment Form: Using this form is the easiest way for proving residency for post season All-Star participation. Please download, complete, and have your child's school Principal sign. Keep it in a safe place until June. Don't Procrastinate, do it while school is in session.

The Parent Code of Conduct is not a Mandatory form. However, it is well worth reading, printing, and signing. All levels of athletics could profit from parents and fans following this code.
